Day care centres

For The Elderly

At Bloom, we are on a mission to combat social isolation amongst the elderly and provide regular safe spaces to come together in your community!

That’s why we offer dedicated Day Care Centres, connecting those living with social isolation, or those living with dementia a space to attend to improve their wellbeing.

Our dedicated team of qualified care staff provide a person-centred care experience offering a day service from 10:00am-16:00pm on weekdays which includes stimulating activities from outside entertainment to crafty creating to movement with music to green fingered gardening to bingo and beyond!

Clients will receive refreshments throughout, a two-course light lunch and the chance to join a new group offering weekly socialisation, quality care and most importantly, a place where people thrive.

We pride ourselves on being able to meet the needs of a variety of individual and therefore our fees vary dependent on level of individual care and support needs but are generally day rated and banded into Band A for low needs, for example those who are self-mobilising and perhaps attending for social inclusion purposes only, Band B for medium needs where some assistance is required with mobility and Band C for high needs whereby support might be required with mobility, behaviour management and personal care whilst with us. Fees are confirmed post pre-assessment stage.

The

Process

Referral

We accept self-referrals from the prospective client or their family or friends and we also accept referrals from healthcare professionals such as Social Workers or GP’s.

Taster session

Following a referral, we will call to discuss our service and extend an invitation to attend a free 2-hour taster session.

Client Pre-Assessment

Following the taster session, all being well we proceed to a full client pre-assessment whereby we look to understand individual care and support needs including medical history, mobility support, dietary requirements, and much more in great detail to ensure we can meet all requirements safely for regular independent attendance.

Client agreement

Once the level of care is ascertained from the assessment, relevant fees are discussed, the client agreement shared and completed and care plans and risk assessments are drawn up in preparation for your first session.

Welcome to bloom

Our final stage of the process is welcoming you to your first official session at our Day Centre, introducing you to our dedicated staff, our other client members and providing you with a day full of fun!
